External rotation in 0 degrees of abduction is a muscle length test for subscap as you highlighted. The work of Bob Donatelli PT and others has looked closely at this test and the relationship with shoulder elevation post injury. By improving range in 0 degrees abduction you will get an improvements in overhead elevation. I have achieved this with my patients by combination of low load prolonged stretch, scapula mobilisations and dry needling of subscap.
